Student Health Forms '25-'26

Dear Davies Families:

I hope the start of the school year is going well for your child. The faculty and I are looking forward to seeing many of you tomorrow evening, Thursday, September 18th, for Meet & Greet between 6pm and 8pm. See my prior communication for the details.

As we continue to settle into the school year, I'd like to bring a couple of important forms to your attention. These forms don't apply to everyone, but you should know about them.

1) Over-the-Counter Medication Permission Form - a completed form is required for any student who would like to receive over-the-counter medication from the school nurse (e.g. Tylenol, Advil, cough drops, etc.)

2) Physical Form - a completed form is required of all current 9th and 12th grade students, as well as all student-athletes; if your child is playing a fall sport, this form is already on file

If either form applies to your child, please print and complete the form(s). You can drop them off tomorrow evening at the school. Copies will also be available and can be completed at Davies.
