Letter from the School Nurse

This letter comes with an update and a couple of reminders.


  • Vision Screening: 


A vision screening is mandated by the Rhode Island Department of Health and Education. Completed at various times throughout the school year, you’ll always be promptly notified of any health issues detected during the screening. 


If you ever don’t want your child to be screened, you must notify the School Nurse in writing. Additionally, you must return appropriate forms completed by your child’s optometrist. Without proper documentation your child will be screened at school. 


  • Physical Exams: 


All Grade 9 students, those entering Grade 12, and individuals either new to Davies or new to the state, are required to have a physical exam. The State of RI Physical Form must be submitted to the School Nurse/Teacher and include Asthma and Allergy action plans if applicable. 


  • Medications: 


Any student that requires a daily medication, an as-needed medication, or who qualifies to self-carry/self-administrate medication during the school day must: 


  • provide the School Nurse/Teacher with an order from a licensed provider with prescriptive privileges. Medication orders can be faxed to: 401-722-1650

  • provide parental authorization to allow the School Nurse/Teacher to administer said medication. The medication authorization form (accompanying this email) can be sent to schoolnurse@daviestech.org 


Medications must be delivered to the nurse and kept in the original container/package as dispensed by the pharmacy. They should never be shared with any other individual. 

Over-the-counter and prescription medications of any kind are not allowed to be brought to school and taken independently by students. All medication must be administered by a nurse.
